The Physicians Choice Advantage and the Physicians Choice Advantage + Rx plan is offered by Marion Polk Community Health Plan Advantage, Inc., a Medicare Advantage Health Maintenance Organization (HMO). These plans are designed for people who are entitled to Medicare Part A and enrolled in Medicare Part B who live in Marion or Polk County in Oregon.

Physicians Choice Advantage + Rx is a comprehensive medical health plan that was developed with the help of the local physicians of Mid-Valley Independent Physicians Association. This Medicare plan include nearly 500 local doctors and extra benefits, giving you the advantage of having a choice!
